Original Research Population Pharmacodynamic Modelof Bronchodilator Response to InhaledAlbuterol in Children and Adults WithAsthma* Kathryn Blake, PharmD; Rajanikanth Madabushi, PhD;Hartmut Derendorf, PhD; and John Lima, PharmD Background: Because interpatient variability in bronchodilation from inhaled albuterol is largeand clinically important, we characterized the albuterol dose/response relationship by pharma-codynamic modeling and quantified variability.Methods: Eighty-one patients with asthma (24% African American [AA]; 8 to 65 years old; baselineFEV1, 40 to 80% of predicted) received 180 ␮g of albuterol from a metered-dose inhaler (MDI),and then 90 ␮g every 15 min until maximum improvement or 540 ␮g was administered; all thenreceived 2.5 mg of nebulized albuterol. FEV1 was measured 15 min after each dose. Thepopulation cumulative dose/response data were fitted with a sigmoid maximum effect of albuterol(Emax) [maximum percentage of predicted FEV1 effect] model by nonlinear mixed-effectsmodeling. The influence of covariates on maximum percentage of predicted FEV1 reached afteralbuterol administration (Rmax) and cumulative dose of albuterol required to bring about 50% ofmaximum effect of albuterol (ED50) and differences between AA and white patients were explored.Results: ED50 was 141 ␮g, and Emax was 24.0%. Coefficients of variation for ED50 and Emax were40% and 56%, respectively. Ethnicity was a statistically significant covariate (p < 0.05). AA and whitepatients reached 82.4% and 91.9% of predicted FEV1, respectively (p ⴝ 0.0004); and absoluteimprovement in percentage of predicted FEV1 was 16.6% in AA patients vs 26.7% in white patients(p < 0.0003). Private experience and observational learning in pharmaceutical demand Tanja Saxell∗† This paper quanties the roles of the physician's own experience and the past choices ofother doctors in pharmaceutical demand. I develop a model of medical decision-makingunder uncertainty about the quality of the match between the patient and drug treatment.Unlike previous demand models, my approach takes into account both private and sociallearning, and allows heterogeneity in product quality across individuals. I test whetherinformation on the past choices of other doctors improves drug choices. Using rich datafrom the market for cholesterol drugs, I show that treatment patterns relying heavily onthe past choices of other doctors can lead to over-prescribing in terms of eciency. Myresults suggest that continuity of care, where a patient is repeatedly consulting the samedoctor, is an ecient policy to limit such behavior.

(February 17, 2016) – Members of the "sandwich" generation can attest to how much time their aging parents spend inside the health care system, because they often accompany them on their succession of visits to the doctor's office, lab, or hospital. Yet despite the fact that on average Medicare recipients spend more than half a month a year (17.1 days) in contact with the health care system, all those visits don't always add up to good care. Depending on where they live, these patients too often don't receive medical care that reflects the best evidence available and, despite progress, many still receive potentially harmful medications, according to a new report from the Dartmouth Atlas Project. The report, "Our Parents, Ourselves: Health Care for an Aging Population," is a review of how adults ages 65 and older, a population predicted to surge from 43.1 million in 2012 to 83.7 million by 2050, receive health care in the U.S., based on 2012 Medicare data. It is also a roadmap for caregivers and patients, especially those with multiple ongoing health problems or dementia. "Our bodies change as we age, and our priorities change, too, as the number of years ahead are fewer than the years behind us," said Julie P.W. Bynum, MD, MPH, associate professor of The Dartmouth Institute for Health Policy & Clinical Practice and the report's lead author. "The information in this report is a good starting point for patients and their caregivers to begin a conversation with their doctor about certain aspects of their care."
